Note: Permit Number 2013030621 1. Missing trellis structures around the building for support of the honeysuckle vines. 2. Remove one of the three co-dominant main leaders in any oak tree where this exists in order to make the tree a Florida Number one or better quality. Make sure the pruning does not leave a stub and that the cut is at the branch collar, otherwise future suckering will occur, making the problem worse. 3. Change out three of the shillings holly that were planted on the south side of the FPL transformer to a variety of shrub that will be a minimum of 30" tall at the time of installation and can grow to maturity to be taller than the transformer (plans called for 30" tall Ilex vomitoria). Modify as-built to reflect this change. 4. Fix any clogged nozzles and adjust to spray onto buildings, windows and pavement is minimized. Need to put a low-angle nozzles on the spray heads in the front of the building that currently are spraying the window. 5. The elm tree that was removed from the front buffer common area due to a utility line conflict needs to be replaced with a 65 gallon size ligustrum tree. Remove the existing almost dead ligustrum that is in the buffer now and plant this new tree in its place. Show this tree replacement on the as-built. 6. All irrigation heads need to be a minimum of 12" away from the building foundation per the Southern Building Code. 7. One muhly grass has settled into a hole by one of the palm trees on the west side of the building. Need to fill the depression and replant the grass plant and remulch any disturbed areas. 8. At next inspection, re-check one of the oak trees on the southeast portion of the site in the south buffer. Trees leaves are totally brown and tree may be dead or partially dead. Replace if tree does not fully leaf out in the next couple of weeks. 9. Need landscape and irrigation as-builts that show any changes to the approved plan as well as the following items: a. Show on the irrigation plan the new spray heads, existing or relocated rotor heads, electric valves and master mainline gate valve that were installed on the east side of the parking lot in the common area of the front buffer. b. Measurements from two fixed points for all valve boxes, wire splice boxes and gate valves. c. Make sure zone numbering on the irrigation as-built agrees with the number on the card in the controller that is located by the Memorial Hospital parking lot. d. Change the relocated tree note in the front common area buffer to reflect the newly planted ligustrum tree as noted in comment 6. 10. Need a card in the irrigation controller that indicates what zones are for Zaxby's and what area of the site and type of heads (spray, rotor or drip) that zone covers. This will eventually need to be done on the same card for the Memorial Hospital zones, the Carabbas zones and the Dollar Tree zones which all use the same water source and controller. 11. Need to replace the shillings holly that are dying along the south property line. Those hollies in that buffer row not adjacent to the existing hedge on the property to the south need to be a minimum of 24" tall to meet current buffer requirements. Note on as-built that the 'schillings' variety of holly were used instead of the standard Ilex vomitoria. Comments That Apply to Building Contractor to Resolve 1. Need to tile out the downspout outfalls to the parking drive so the water does not wash out the mulch or drown the plants. 2. All wall pack lighting are required to have glare shields per Land Development Code Section 9.07.05.B Full Cutoff Light Fixtures which states, "Luminaires shall be designed to ensure that no candela occurs at or above an angle of ninety (90) degrees above the horizontal plane (nadir). This applies to all lateral angles around the luminaire. Such candela information shall be determined by a photometric test report (plan) form the lighting contractor. Any structural part of the luminary for all types of lighting providing cutoff angle shielding shall be permanently attached, except that accent uplighting for trees and building walls may be allowed to exceed the ninety (90) degree angle plane, subject to review and approval by the Land Use Administrator if determined to provide an equivalent public health benefit." Need to provide shields on existing fixtures or change out fixtures to comply. This will not hold up the C.O. of the building, but will result in a code warning notice violation if not addressed within a reasonable amount of time.
