Dock is being occupied / used in violation of the Florida Building Code and Florida Fire Prevention Code. Immeadiately cease and desist use of the dock facility until such time as this permit and permit for fire protection are finaled and closed out and Certificate of Completion is issued for both permits. The permit has previously been extended on two different occasions. The permit will not be extended a third time as it has been issued and active for one full year and owner and contrator were informred at time of issuance that a separate permit was required for installation of the fire protection features required by the Florida Fire Prevention Code. No progress has been made to obtain the fire protection permit, no inspections have been scheduled for the dock structure and no valid reason has been provided to justify a third permit extension.

On calculations cover sheet provide digital seal and signature in accordance with Florida Statute 471.025 and Florida Administrative Code 61G15-23.004. Plans transmitted to the City require digital seals and signatures per these referenced regulations. 2. Provide Florida licensed professional engineer seal on all plan sheets SD1 through SD5. Signature and seal to comply with Item No. 1 above as plans have been transmitted electronically. 3. Provide Florida license number of engineering firm in the Title Block per F.S. 471.023 and FAC. 4. Note that plan sheet SD5 is missing. 5. On resubmittal uploade calculations separately as one file and upload each engineering plan sheet separately (1 sheet per file/upload). 6. Provide details for accessible gangway showing compliance with the 2020 Florida Building Code-Accessibility Section 405. This includes slope, edge protection, handrails, extended floor surfaces, materials for wet conditions, clear ramp width, maneuvirng clearances at turns, landing dimensions and slopes, etc. Note that Plan Sheet SD1 refers to Plan Sheet SD5 for handrail details; however Plan Sheet SD5 is missing. 7. Correct project site information in Title Block on each plan sheet to refelct that the proect is in the City of Palm Coast. Provide street address as well. 8.
