C2d may be left as N/A, staff did not realize this addition was connected to the rest of the structure by no more than a walkway. C2e cannot be left blank if the Comments in Section D state that the equipment has not yet been installed and the garage is still under construction, but a final survey was submitted. It seems that the Foundation Elevation Certificate was resigned and resubmitted, please submit the Final on the new form with revisions.
